What you can do outside training

The exercises are one half. The other half is how you sleep, what you eat, how much you sit and how tense you are. Without these, even the best programme delivers only part of the result.

Sleep - the fastest lever

Most testosterone is produced during the REM phases of sleep. One week of 5-hour nights measurably reduced daytime testosterone in young men, by 10 to 15%. Sleep apnoea (snoring plus daytime fatigue) is itself a common underlying cause.

  • Aim for 7-8 hours, ideally with a consistent bedtime.
  • Put the phone down 90 minutes before bed and dim the lights.
  • If you snore loudly and wake up tired, ask for a sleep study - that is a medical question.

Diet - fuel for vascular function

The strongest evidence supports the Mediterranean pattern: several studies found it improved erectile function scores. The logic is simple - what is good for your arteries is good for your erections.

  • Vegetables and fruit daily, legumes several times a week.
  • Olive oil as the main fat; oily sea fish twice a week.
  • Cut back on processed meat, sugary drinks and refined flour.
  • A handful of nuts a day - in several studies this alone improved outcomes.

Body weight and waist circumference

Abdominal fat converts testosterone into oestrogen via the aromatase enzyme and sustains an inflammatory state that damages vascular function. In overweight men, even a 5-10% loss of body weight improved erectile function.

  • Waist circumference is a better marker than weight: above 94 cm it is worth acting.
  • Do not starve yourself - very low calorie intake also lowers testosterone.
  • Strength training preserves muscle mass while you lose fat.

Smoking and alcohol

Nicotine causes immediate vasoconstriction and damages the endothelium long-term. Men who smoke have roughly double the risk of erectile dysfunction. Alcohol in small amounts lowers inhibition, but in larger amounts directly impairs erection, and heavy regular drinking lowers testosterone.

  • Vascular function improves measurably within months of quitting, especially under 50.
  • Keep alcohol to 1-2 occasions a week, at most 2 units each time.
  • Vaping is not a neutral alternative: the vascular effect of nicotine is the same.

Stress, anxiety and performance pressure

An erection is a parasympathetic process: it works when the body feels safe. Fear of failure alone can sustain the problem - and this vicious circle is often stronger than the original cause.

  • Five to ten minutes of diaphragmatic breathing a day is the simplest intervention.
  • Talk to your partner about it - secrecy increases the anxiety.
  • If anxiety dominates, sex therapy is proven effective and combines well with the exercises.

Cycling and your saddle

Cycling is not the culprit - the wrong saddle is. A narrow, nosed saddle presses on the perineum, compressing nerves and blood vessels. Several hours a week on one can cause numbness and temporary erectile problems.

  • Choose a wide, noseless or centre-cut saddle.
  • Set the saddle level and stand up out of it every 10 minutes.
  • Numbness in the perineum is a warning sign - adjust your setup immediately.

Desk work and inactivity

Eight or more hours of sitting a day shortens the hip flexors, inhibits the glutes and impairs pelvic circulation. Prolonged sitting is an independent risk factor, regardless of whether you train.

  • Stand up for 2-3 minutes every hour - set a reminder.
  • Stretch your hip flexors at least once a day.
  • Seven to eight thousand steps a day already makes a measurable difference.

Medicines that may contribute

Erectile dysfunction is a side effect of many common medicines - but never stop taking them on your own. Talk to your doctor: there is often an alternative.

  • Certain blood pressure drugs (beta blockers, thiazide diuretics).
  • SSRI antidepressants.
  • 5-alpha reductase inhibitors taken for prostate enlargement or hair loss.
  • Some antacids, sedatives and muscle relaxants.

What not to do

Do not order “potency boosting” supplements online: regulators repeatedly find undeclared pharmaceutical ingredients in them, which can be life-threatening together with heart medication. Do not take medication prescribed for someone else. And do not wait years - a persistent problem needs to be investigated.
